Here is a breakdown of our enrollment process and next steps:
1. Call today to schedule an initial assessment. Take a tour of our facility while our trained staff members answer all of your questions and gather the information necessary to determine the eligibility of your child.
2. Once the initial assessment has been completed, we will require a physician’s order to enroll your child in our program.
3. Upon receiving a physician’s order, a Plan of Care will be developed based on your preferences and that of your physician.
4. Once your child has been enrolled, we will work with you to assist in coordinating your special needs child to and from our location with provider-supervised transportation.
